We are searching for a dedicated Ambulatory Care Pharmacist to join our dynamic healthcare team. In this role, you will play a crucial part in optimizing medication therapy for ambulatory patients, conducting comprehensive medication reviews, and collaborating with healthcare providers to ensure the highest quality of patient care. The ideal candidate will possess a background in ambulatory care pharmacy or a willingness to learn, a commitment to patient well-being, and excellent communication skills.
Key Responsibilities:
Conduct comprehensive medication reviews for ambulatory patients to identify, resolve, and prevent medication-related problems.
Develop personalized medication action plans and ensure proper medication management and adherence.
Collaborate with healthcare providers to optimize medication therapy and enhance patient outcomes.
Provide patient counseling and education on medication use, adherence, and potential side effects.
Monitor patient progress and adjust medication therapy as needed to achieve optimal therapeutic results.
Utilize electronic health records (EHR) to document interventions and track patient outcomes.
Participate in the development and implementation of clinical protocols and guidelines for ambulatory care.
Stay current with best practices and advancements in ambulatory care pharmacy.
Assist with transitions of care to ensure continuity and safety of medication therapy.
Engage in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.
Preferred Qualifications:
Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program.
Experience in ambulatory care pharmacy.
State pharmacy license in good standing.
Proficiency in utilizing electronic health record (EHR) systems.
Strong analytical and problem-solving skills.
Excellent communication and collaboration abilities.
Board certification in ambulatory care pharmacy (e.g., BCACP) preferred.
Familiarity with chronic disease management and preventive care strategies.
