Job Description

Assignments may include the following:

• Assist in the assessment of capacity and needs of the individual who reside at the center
• Conduct behavioral assessments (Vineland, Vanderbilt Assessment Scale)
• Develop and implement treatment plans and keep detailed records
• Develop, implement and monitor behavioral plans, providing individualized instructional and behavioral support plans and assisting in crisis situations
• Monitor and record progress towards goals
• Conduct behavioral analyses to identify triggers, frequencies and consequences of an individuals behavior
• Facilitate skill building sessions to help individuals develop social skills, self care skills and other life skills
• Collaborate with the interdisciplinary team (psychiatry, PSS, RSA/RSAT, PSD, FD, CDS and other healthcare professionals)
• Participate in team meetings and case reviews
• Support transition activities of individuals from the state center to the community
