Summary:
21-1020 Social Workers-2 Performs entry level protective services casework to administer services, provide consultation, case management, education and guidance for youth, their families, colleagues and the community to assure that all adolescents acquire the preparation, resources and positive youth development needed to establish connections with their community and others in order to achieve self-sufficiency and success as adults while promoting the prevention of child abuse and neglect. Upholds the vision and mission of the agency through utilizing the Practice Model theories, beliefs and principles in daily practice.
21-1020 Social Workers-3 To perform increasingly complex protective services casework to coordinate, manage and administer services according to case plan to prevent abuse/neglect of children and to work with adolescents and their families in order to provide support and services. To consult, educate, train and assist DCYF staff in order to meet the needs of DCYF involved adolescents while promoting the prevention of child abuse and neglect. Upholds the vision and mission of the agency through utilizing the Practice Model theories, beliefs and principles in daily practice.
21-1020 Social Workers-4 To perform protective services casework to coordinate, manage and administer services according to case plan to prevent abuse/neglect of children and to work with adolescents and their families in order to provide support and services. To consult, educate, train and assist DCYF staff in order to meet the needs of DCYF involved adolescents while promoting the prevention of child abuse and neglect. Upholds the vision and mission of the agency through utilizing the Practice Model theories, beliefs and principles in daily practice.

MINIMUM QUALIFICATIONS:
Education/Experience : Equivalent combination of 2.5 years of education and experience after completion of high school. Education and experience must be in social work, professional case management, psychology, social psychology, sociology, human services, criminal justice, behavioral science, cultural anthropology, counseling, pastoral counseling, divinity, education, or a related field.
License/Certification: Valid driver’s license or access to transportation

Responsibilities:
21-1020 Social Workers-2
Possesses basic knowledge of the principles of case management in order to manage a caseload and prepares and develops Solution Based case plans in conjunction with the youth and their family to ensure that objectives and tasks are being achieved that court orders are being complied with, when applicable, in accordance with State and Federal Statute.
Provides consultation, education and training to District Office staff, collaterals and providers regarding the Adult Living Preparation process and the opportunities and resources available for adolescents through the DCYF Adolescent Program, under the direction of the supervisor
Actively participates in Permanency Planning Teams (PPT) meetings to ensure adolescents do not leave care without adequate adult living preparation, a viable permanent connection and that permanency is achieved in a timely manner.
Assists District Office staff with the Adult Living Preparation Process to ensure that all eligible youth have this process completed and provides case management services to young adults receiving Aftercare Services.
Assists residential facilities, group homes and child placing agencies with the Adult Living Preparation Process by providing necessary forms and materials.
Ensures that youth in placement obtain the preparation, resources and positive youth development they need to establish connections with caring adults with the goal to become healthy, self sufficient and successful adults.
Connects DCYF (CPS & JJS) involved youth to the DCYF Advisory Board, the DCYF Youth Conference and other conferences, trainings, and positive youth development opportunities.
Completes all required documentation, including case plans, court reports and all necessary Bridges documentation of case related activities.
Actively participates in internal and external committees relating to adolescent work.
Works under direct and close supervision and receives regular scheduled weekly supervision.
Completes training requirements according to policy including individual training plan and 30 hours of in-service training annually after completion of Child Protective Service Worker (CPSW) Academy.
