Job Description
We are seeking a compassionate and experienced Community Health Worker to provide advocacy, outreach, and support for individuals in underserved communities. This role serves as a vital member of a multidisciplinary team focused on promoting health and wellness. The position involves facilitating health education, disease prevention, and wellness promotion across various communities.
Responsibilities
+ Facilitating health education, disease prevention, and wellness promotion.
+ Supporting patients in navigating healthcare systems and accessing needed services.
+ Assisting clients with resolving personal challenges and connecting to community resources.
+ Providing basic counseling and support for individuals managing medical and social needs.
+ Leading and organizing community outreach events and group activities.
+ Supporting enrollment in programs such as Medi-Cal, Medicare, and other state, county, and federal services.
+ Reducing barriers to care and increasing access to health and social services.
Essential Skills
+ Ability to work flexible, non-traditional hours to attend community events.
+ Strong interpersonal and communication skills.
+ Experience working with diverse populations and underserved communities.
+ Knowledge of local health and social service resources.
+ Ability to work independently and collaboratively within a team.
+ Familiarity with medical terminology, HIPAA regulations, and proper handling of Protected Health Information (PHI).
Additional Skills & Qualifications
+ Bilingual proficiency preferred in Spanish, Punjabi, or Dari.
+ Proficient in clerical tasks, customer service, and personal support with a strong command of the English language.
+ Skilled in communicating effectively with supervisors, peers, and subordinates, as well as external partners and community members.
+ Comfortable using computers and digital systems to process information, document records, and stay updated with relevant knowledge.
+ Capable of resolving conflicts, negotiating solutions, and organizing, planning, and prioritizing work efficiently.
+ Experienced in establishing and maintaining interpersonal relationships across diverse populations.
+ Proven ability to work with a wide range of individuals in multicultural settings, fostering trust and collaboration.
+ Working knowledge of Medi-Cal, CalFresh, and other public assistance programs is preferred.
+ Understanding of multi-system outreach programs related to healthcare delivery, patient education, and health-related services.
+ Highly capable of organizing resources, setting priorities, and managing tasks efficiently.
Work Environment
Position will work from a clinic but occasionally may travel to designated locations as needed. Must be able to travel to different clinic sites and community events. Flexibility in work schedule is required, as this position will need to attend weekend or evening events. The work environment is team-oriented, promotes long-term career growth, and offers opportunities for team achievements and partnering for growth.

This is a Permanent position based out of Marysville, CA.
Pay and Benefits
The pay range for this position is $59571.20 - $59571.20/yr.
