Job Description

GENERAL PURPOSE

To provides quality pharmacy consulting services to long term care residents that maximizes therapeutic interventions, educational programs, and knowledge of pharmacy laws along with state and federal regulations to produce overall positive patient outcomes.

****This position will require you to live in the North Alabama area.***

QUALIFICATIONS

· Bachelors degree from an accredited School of Pharmacy or PharmD

· Current licensure in the state(s) of practice

· Current consultant license in state of practice (if applicable)

· 1 year consulting experience in long-term-care environment, preferred.

· Strong analytical skills; ability to quickly identify problems and find effective solutions.

· Strong written and oral communication skills.

· Basic computer skills; experience with Microsoft Excel and Word preferred.

· Must be able to work extended hours

· Must be able to travel, including overnight.

· Ability to make independent decisions.

· Ability to understand and interpret financial data.

· Ability to multitask and complete jobs without supervision.

· Ability to assess information and make prudent, professional decisions.

· Ability to work and produce results under pressure

· Ability to effectively and harmoniously interact with facility, management and other employees at all levels; promotes teamwork and professionalism

· Must be capable of maintaining regular attendance

· Must be dependable and able to work with minimal supervision

· Must be at least 21 years of age

· Must be able to speak, read, write, and understand English

· Must be capable of performing the ESSENTIAL JOB FUNCTIONS and PHYSICAL AND SENSORY REQUIREMENTS FOR ALL JOB FUNCTIONS as outlined below.

STANDARD REQUIREMENTS

· Maintains confidentiality of all patient and/or employee information to assure patient and/or employee rights are protected.

· Support the Mission, Values, and Vision of Senior Care Pharmacy.

· Report to Management immediately all instances of misappropriation/handling of company resources.

· Support, cooperate with, and implement specific procedures and programs for:

o Maintaining confidentiality of all data, including resident, employee, and operations data.

o Safety and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs.

o Compliance with all regulatory requirements

o Compliance with current law and policy to provide a work environment free from harassment, and all illegal and discriminatory behavior.

· Support and participate in common teamwork:

o Cooperate and work together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.

o Use tactful, appropriate communications in sensitive and emotional situations.

o Follow up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems, and concerns.

o Promote positive public relations with residents and family members.

o Complete requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned.

· Provide great customer service to internal and external customers:

o Promote positive public relations with residents, family members, and guests.

o Support Senior Care Pharmacy in a friendly, courteous, and cooperative manner.

· Comply with all company policies and procedures regarding HIPAA Privacy and Security.

ESSENTIAL JOB FUNCTIONS

· Performs and/or oversees the completion of all routine and tasks, including but not limited to:

o Performs a Monthly Medication Review for each resident within an assigned LTC facility at least monthly and reports irregularities and/or concerns to the Director of Nursing, Administrator, and the attending physician through a written report of findings and recommendations.

o Evaluates, through the Monthly Medication Review, physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration based on documented diagnosis, lab results, assessment of resident’s ability to swallow, and other pertinent healthcare information.

o Implements corporate clinical initiatives derived through SCP corporate collaboration with P&T committee.

o Works with Director of Clinical Services to implement and manage cost containment programs, including DURs.

o Applies knowledge of pharmacy laws as well as state and federal laws governing long term care pharmacy services into monthly consultant reviews for assigned LTC facility.

o Assist Director of Clinical Services with account management, client services,

and customer service issues.

o Facilitates the development, implementation, and maintenance of systems that promote safe, effective, and cost efficient medication management thus providing optimal patient care.

o Educates physicians, nursing staff, pharmacy staff, and other health care professionals through in-services regarding regulatory, clinical, and other pertinent health care information, resulting in improved patient care practices.

o Works cooperatively with the facility and the pharmacy as a health team member.

· Accepts ownership and accountability for the successful completion of assigned functions and achievement of goals.

· Strives for compliance by all staff with internal controls and state and federal regulations.

· Participates in the development and successful implementation of plan(s) of improvement as indicated, recommended and/or required by Supervisor, Director, or any other corporate or regulatory compliance consultant. Communicates progress toward goals and ongoing obstacles to supervisor and company management team, verbally and/or in writing or as dictated.

· Provides training to other staff that is necessary to successful job performance, consistent with company standards and expectations, and facilitates cross-functional awareness and proficiencies.
