Job Description

POSITION/JOB SUMMARY:

  • The Nurse Diabetes Educator provides expert clinical guidance, education, and support to patients with diabetes, their families, and the care team at Johnson Memorial Health Services. This role supports both inpatient and outpatient services, promoting optimal diabetes management and prevention strategies in alignment with current clinical guidelines and best practices.
  • Responsible for the delivery of safe, effective, and quality patient-family centered care in the Outpatient and Perioperative Services, for all patient populations. 

This position is regularly scheduled four days per week with the potential to be called in if available for weekend surgical procedure.

ESSENTIAL FUNCTIONS

  • Deliver diabetes self-management education and support (DSMES) for patients with Type 1, Type 2 and gestational diabetes.
  • Collaborate with medical providers to create individualized diabetes care plans.
  • Serve as a resource to hospital staff on diabetes medications, insulin therapy and device usage.
  • Coordinate outpatient education programs and community diabetes awareness events.
  • Document patient interactions and outcomes accurately in the EMR.
  • Ensure compliance with ADA and AADE standards.
  • Support continuity of care between hospital and outpatient settings.
  • Stay current on diabetes care guidelines, research, and technology.
  • Assesses patients at intervals of care as stated in policies and procedures.
  • Implements the plan of care by:
    1. Protecting the patient from injury caused by chemical, electrical, mechanical, and thermal sources and extraneous objects.
    2. Performing counting procedures to protect the patient from unintended retained surgical items.
    3. Performing interventions necessary to ensure that the patient’s procedure is performed on the correct site, side, and level.
    4. Managing the patient’s specimens in the appropriate manner.
    5. Communicating the patient’s current status to the appropriate parties throughout the continuum of care.
    6. Administering medications safely and correctly.
    7. Performing interventions to maintain the patient’s wound and tissue perfusion at or above baseline levels.
    8. Performing interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period.
    9. Performing interventions to protect the patient from surgical site infections.
    10. Assessing the knowledge level of the patient or designated support person.
    11. Providing education regarding the expected psychosocial response, nutritional management, medication management, pain management, wound management and expected responses to the operative or invasive procedure.
    12. Actively involving the patient or designated support person in decisions affecting his or her perioperative plan of care and the rehabilitation process.
    13. Protecting the patient’s rights, dignity and privacy.
    14. Providing culturally and age-appropriate, ethical care within legal standards of practice.
    15. Knowledgeable in asepsis and sterile techniques.
    16. Demonstrates knowledge of instruments and equipment used in the operating room including specialty equipment as it applies to the surgical specialty services being provided at JMHS.
    17. Delegates tasks as appropriate according to the MN Board of Nursing Practice Act
    18. Documents nursing care accurately, completely and legibly.
    19. Collaborates effectively with other disciplines as appropriate.
    20. Participates in quality review and performance improvement projects.
    21. Uses problem-solving and conflict resolution skills to foster effective work relationships with team members.
    22. Maintains required competencies.
    23. Acts as a patient advocate and maintains

    • Follow up as appropriate with supervisor, co-workers, patients, or residents regarding reported complaints, problems, and/or concerns.
    • Promotes positive public relations with patients, residents, family members and guests.
    • Completes requirements for in-service and annual training, acceptable attendance, uniform dress codes including personal hygiene, and other work duties as assigned.

QUALIFICATIONS

  • Certified Diabetes Care and Education Specialist (CDCES) – or eligible within 2 years
  • Minimum 2 years of clinical nursing experience
  • Graduate from an accredited school of nursing
  • Current registered nurse licensure in the State of Minnesota
  • Current Basic Life Support (BLS) certification or must be obtained within thirty (30) days of hire
  • Current Advanced Cardiac Life Support (ACLS) certification or must be obtained within six (6) months of hire
  • Pediatric Advanced Life Support (PALS) certification or must be obtained within six (6) months of hire

WORKING ENVIRONMENT

  • Able to stand, walk, bend, squat, reach, and stretch frequently.
  • Must use appropriate standard/isolation precautions due to threat of exposure to blood and body fluids.
  • May be exposed to patients with infectious diseases.
  • Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.
  • Must be able to lift up to 50 pounds at times.
