Thrive By 5 is currently hiring Service Coordinators for the Early Intervention Program in Staten Island! This position involves providing continuous and interactive services that ensures the successful participation of eligible children aged 0-3 and their families in a home-based and/or community-setting. 

Essential Duties and Responsibilities:

The essential duties and responsibilities are not limited to the following:

• Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
• Ensure the IFSP outcomes and strategies reflect the family's priorities, concerns, and resources; and that changes are made as the families/child's needs change.
• Coordinate the provision of all services that the child needs and/or are receiving.
• Facilitate the timely delivery of services.
• Continuously seek appropriate services and situations that benefit the development of the eligible child for the duration of the child's eligibility.
• Perform related duties as required.

Qualifications, Knowledge and Experience:

• Bachelor's Degree in health and/ or human services field.
• Ability to effectively communicate with clients and families via phone and email.
• Previous experience preferred
• Case management experience preferred
