Position Summary :  As a Hospice Home Health Aide, you will function under the supervision of the RN Case Manager and Hospice Aide Manager as part of an interdisciplinary team. The role of the Hospice Aide is to provide delegated health care tasks in a patient’s home including:  Personal hygiene, Assist patients in performing activities related to mobility such as transfers, ambulation, turning, and repositioning; Obtain vital signs; Perform simple dressing changes, Light housekeeping and other related support services essential to the client’s health; Observe, record and report all changes to the supervisor.  The goal is to assist each patient in achieving the highest level of comfort and quality. 

Job Duties:

  • Provides personal cares as detailed in the plan of care
  • Displays an attitude of courtesy and respect for patients, families, staff and referral sources
  • Seeks and accepts feedback regarding the performance of job duties
  • Is aware of patient confidentially and adheres to HIPPA regulations
  • Displays an understanding of vulnerable adult issues
  • Displays an understanding of hospice patients’ rights
  • Completes all required documentation in timely manner
  • Demonstrates a desire to work with the terminally ill

Interpersonal Relationships

Patient Care Delivery

  • Possesses strong problem solving skills.
  • Possesses the knowledge, skills and abilities to perform selected Home Health / CNA procedures to patients in their home and specific delegated techniques for patient care.
  • Works with other disciplines and works as a good team member.
  • Excellent organizational and time management skills.
  • Utilizes laptop computers/iPAD for client documentation while completing charting as specified in policy.
  • . Assists the registered nurse with data collection in collaboration with the resident, family and/or significant others and the interdisciplinary health care team to meet patient specific plan of care.

Qualifications:

  • Listed on MN nursing assistant registry with current Nursing Assistant Certification (CNA).
  • 1-year experience as a home health aide or nursing assistant.
  • Current valid MN Driver’s License and daily access to reliable, insured vehicle.
  • Ability to use and/or learn electronic charting on tablet and/or mobile phone.
  • Demonstrates a desire to work with the terminally ill.
  • Ability to demonstrative positive and respectful communication/interaction skills.
