Overview: The Wellness Center Social Worker works as part of a healthcare team to assess, counsel and provide resources around students' personal and psychosocial issues. Responsibilities: Patient Interaction/intervention: Identify and determine the appropriate level of intervention in a timely manner. Insure appropriate alignment of services to students, as needed. Follow all legal mandates, regulations and professional ethical standards. Chart Documentation: Provide a concise written record of patient care and counseling that is inclusive of dates, patient assessment, and plan of care. Complete the Billing and Evaluation encounter form for each visit using accurate CPT codes, diagnosis codes and tracking-referral codes. Psychosocial Assessments: Demonstrate knowledge and utilization of screening tools for depression, anxiety, trauma and post traumatic stress disorder (PTSD) and other conditions encountered in the school-based setting. Environment: Demonstrate the ability to adapt to changing situations and provide versatility in prioritizing cases. Triaging a patient as a "whole person" is important. Maintain an atmosphere within the School Based Wellness Center (SBWC) where each patient feels safe. Protection of patient confidentiality is of the utmost importance. Respond to requests from the school community for interventional and educational group therapy sessions. Utilize outside agencies and resources to support group sessions, as needed. Engage in other duties as deemed necessary to the daily operations of the School-Based Wellness Center. Qualifications:

Previous Social Work experience preferred. Strong written communication skills required for proper documentation on the medical record. Possess strong time management skills able to prioritize tasks for crises management. Must be skilled in interviewing techniques; possess skills necessary for conducting group sessions and professional counseling.
