Job Description

Collect, Scan, Upload, and File Records
  • From Medical Records Basket

Records Requests
  • Respond and generate records request from patient, insurance and doctors office for continuity of care.

Disassemble/Audit Discharge Charts
  • Collect all Discharge Records
  • Scan and upload all orders/records pertaining to discharge.
  • Make sure all discharge charts are removed from the station

Assemble Charts for New Admissions
  • With 9 Tabs (Admission Records, Advanced Directives, Consents, Physician Orders, History and Physical, Assessments, Social Services, Hospice, & Legal Documents)

Chart Organization
  • Make sure all patient charts with the same name have "name alert" sticker
  • All room numbers are correct and in place.
