Are you a dedicated healthcare professional passionate about delivering personalized, compassionate care? Do you thrive in environments where you can truly make a difference in patients' lives? If so, we're looking for you!

This role offers a unique opportunity to provide exceptional medical care to individuals, primarily those living with Chronic Kidney Disease (CKD) and/or End-Stage Renal Disease (ESRD/ESKD). You'll play a vital role in enhancing their quality of life by conducting thorough assessments, developing personalized care plans, and providing ongoing support and education.

In this position, you'll be empowered to prioritize patient well-being, focusing on delivering evidence-based care that reduces avoidable hospitalizations and maximizes patient satisfaction. If you are looking to truly make a difference, this is the role for you.

Why You'll Love This Opportunity:

Flexible and Hybrid Work Environment: Enjoy the flexibility of a hybrid and in-home work model, allowing for a better work-life balance.
Patient-Centered Care: Practice value-based care, where you can dedicate quality time to your patients and build meaningful relationships.
Competitive Compensation and Benefits: Receive a competitive salary aligned with MGMA guidelines, along with comprehensive medical, dental, vision, and life insurance.
Generous Time Off: Benefit from paid vacation and holiday time to recharge and enjoy personal time.
Retirement Savings: Secure your future with a 401(k) plan with matching contributions.
Relocation Assistance (Case Dependent): For eligible candidates, we offer paid relocation assistance to help you make a smooth transition.
Focus on Improving Lives: Join an organization that prioritizes patient well-being and strives to improve health equity for individuals with chronic kidney and end-stage renal disease.
Your Responsibilities:

Conduct comprehensive assessments, including annual wellness exams, in patients' homes and virtually.
Provide patient and family education on available benefits and programs to promote healthier living.
Maintain accurate and detailed patient records, including medical history, diagnoses, and treatment plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Deliver timely, evidence-based care to minimize hospitalizations and enhance patient quality of life.
Prescribe medications, order tests, and collaborate with physicians to develop and implement treatment plans.
Educate patients on preventive monitoring and schedule diagnostic testing to close gaps in care.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Make assessments of patients health status, develop treatment plans, and implement and evaluate those plans.
Other duties as assigned.
What We're Looking For:

Active and unrestricted Registered Nurse and Nurse Practitioner (NP) or Physician Assistant (PA) license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Proficiency with medical instruments, equipment, and computer-based data management systems.
Excellent verbal and written communication skills.
Strong understanding of HIPAA and OSHA regulations.
Minimum of 2 years of direct patient care experience.
Experience in managed care, IPA, or health plan settings is preferred.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home is highly valued.

Employment Type: Full-Time
Salary: $ 129,872.00 Per Year
