• Perform Initial Assessments for new clients either at the start of care or within 24 hours
• Develop a plan of care for each new client
• Perform Care Monitoring Visits on current clients every 60 days or less
• Complete all necessary initial evaluation documentation in compliance with CAHC regulations
• Manage end of service/discharge of clients
• Support caregivers in the field with training as needed
• Perform caregiver evaluations and start of first case evaluations as necessary
• Communicate with nursing team/ DON via email as needed
• Weekly Orientation/skills lab for home health aides upon hire
• Document any billable services within 24 hours
• Participate with on-call rotation 1 weekend per month

• Bachelor of Science in Nursing degree preferred.
• Valid NJ RN license.
• Valid NJ driver’s license.
• 1-2 years of field nursing experience in a Homecare setting
