Key Responsibilities:
Review and evaluate physician medication orders to ensure the appropriateness of drug choice, dosage, potential interactions, and route of administration.
Collaborate with facility staff and physicians to recommend adjustments to therapy as necessary.
Conduct regular reviews of resident drug regimens, reporting any irregularities to facility leadership with detailed findings and recommendations.
Maintain thorough documentation of all communications with healthcare teams, residents, and their families.
Stay up to date with federal and state regulations related to long-term care pharmacy services.
Work closely with care teams to guide medication management strategies that optimize patient care and reduce costs.
What You'll Bring:
A Bachelor's degree in Pharmacy or a PharmD from an accredited institution.
A current pharmacist license (dual licensure is a plus).
Experience in acute care, long-term care, or home health settings is preferred.
Experience as a Consultant or Clinical Pharmacist is ideal.
IV clinical experience is a bonus.
Willingness to work occasional weekends or outside normal business hours.
Strong communication and teamwork skills.
Familiarity with state and federal long-term care regulations, along with formulary management.

Requirements:
Immunization certification (e.g., from APhA) or willingness to obtain within 90 days.
Ability to lift and handle patient charts repetitively.
Willingness to stand for extended periods during shifts.
This position is remote and covers the Pennsylvania region. Applicants must reside within the specified territory to be considered.
