Job Description

The Mission Neighborhood Health Center is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services.  We advocate for health equity and deliver innovative, high quality services responsive to the neighborhoods and diverse communities we serve with a focus on the Latino Spanish-Speaking Communities.

We are currently looking for qualified candidates for our Senior   Health Educator role. This position reports to the  Wellness Director  and is a Regular, Full Time, Exempt role with a starting annual range of $75,400 to $83,000 with full benefits. 

The primary objective of this position is to design targeted health education initiatives aimed at promoting healthy lifestyles and wellness among our patients and community. . The health educator will provide individual and group education and counseling about preventative care guidelines, chronic conditions, healthy lifestyles, and treatment education and medication adherence. The Health Educator is responsible for developing general health education and wellness campaigns in the community.  Additionally, the health educator will educate patients on the use of telehealth technology and the use of the patient portal.

Essential Functions & Responsibilities

The Health Educator has the following responsibilities and duties:

Patient Care (65%)

  • Conduct health education with patients and their families; Provide individual and group health education counseling sessions to MNHC patients on a variety of topics (e.g. chronic conditions such as asthma, Diabetes, hyperlipidemia, etc., self-management of these conditions, preventative care guidelines and the adoption of healthy lifestyles).
  • Conduct individual interviews to assess patients’ knowledge of their chronic conditions, belief systems and health literacy capacity, as related to health, illness, and health maintenance areas (e.g. medication, diet, exercise, dental health, stress, asthma, substance use, and sexual behaviors, if appropriate).
  • Provide Enhanced Care Management (ECM) support to patients with chronic conditions—particularly asthma—by offering structured self-management guidance and coordinating care interventions as part of a multidisciplinary team.
  • Support patient’s self-management of chronic conditions through the use of at home remote patient monitoring technology.
  • Follow the recommendations, best practices, and guidelines of the Center for Disease Control and Prevention contained in the CDC’s Chronic Disease Prevention and Health Promotion website.
  • Serve as a health education resource across all MNHC clinic sites, ensuring consistent, culturally relevant educational messaging and materials.

Patient Wellness and Health Promotion (15%)

  • Under the direction of the Wellness Director, lead and organize National Health Observances campaigns and activities for MNHC patients that are relevant to the patients and communities served by MNHC.   
  • Coordinate with other staff members and implement Wellness and Health Promotion Activities among MNHC patients, such as Walking Campaigns, distribution of healthy snacks and health promotion group activities
  • Support patients and medical providers in the use of telehealth technology and patient portal to ensure equitable access among our patient population.

Community Health Promotion (10%)

  • Provide health education classes at community venues where target population congregates or accesses services (e.g. social service agencies, churches, etc.) in coordination with community partners, as requested. Health education topics will include chronic conditions and its management , healthy lifestyles options, etc.
  • Attend community events (e.g. health fairs) in coordination with other MNHC staff to provide health promotion materials to target population.

Administrative (10%)

  • Contribute content on community health education and asthma-related outreach for the Mission Promise Neighborhood (MPN) quarterly narrative reports.
  • Conduct assessments and surveys to determine health education needs of our patient population.
  • Translate scientific language and concepts into clear, simple and understandable information for patients to maximize the potential for them to take reasonable action
  • Help develop, identify, implement and improve patient education materials and other tools that encourage healthy decisions.
  • Accurately chart all patient assessments, interventions, and referrals in the electronic medical record.
  • Submit monthly reports, statistical data, program reports and audits as requested by supervisor.
  • Keep informed of new scientific developments in chronic conditions and treatments, as well as health promotion and injury prevention.

Core Competencies
Demonstrated knowledge of principles, methods and materials used in health education and health promotion.

  • Knowledge of chronic conditions (e.g. diabetes, hypertension, obesity, hepatitis, etc.) and treatment options.
  • Knowledge of preventative care guidelines contained in the CDC’s Chronic Disease Prevention and Health Promotion website
  • Excellent interpersonal skills, verbal and written communication skills and computer skills are essential.
  • Group facilitation, presentation and organizational skills required.

Qualifications & Requirements

The Health Educator should have the following skills, education, and experience:

Education

  • Bachelor's degree in health education or public health.
  • Certified Health Education Specialist preferred.
  • Bilingual – Written and oral Fluency in English/Spanish required.
  • Certified Asthma Educator (AE-C) preferred.

Experience

  • Minimum of 3 years providing health education services to diverse populations.
  • Ability to work professionally and ethically within multi-cultural settings, including ability to maintain confidentiality and privacy of persons, documents, data, and communications.
  • Flexibility in scheduling, weekends and evening work will be required.
  • Experience working with Latino population.
  • Experience working in Primary Care setting preferred.

Physical Demands:
While performing the duties of this job the employee is required to sit approximately 60% of the time, walk 20% of the time and stand 20% of the time. The employee is frequently required to use hands and fingers to type or handle, feel, and operate objects. The employee is required to occasionally lift and/or move up to 10 pounds. While performing the duties of this job the employee is occasionally required to reach with hands and arms. The employee is frequently required to speak and listen. Specific vision abilities of this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
