Role: Community Health Advocate 
Location: New York, NY 
Duration: 13+ Weeks
Guaranteed Hours: 37.5 Hours
Hours per day: 7.5
Shift: 8:30am-5:00pm
Pay Rate: Local - $18.00-$21.00/Hour


Roles & Responsibilities:

• Provides direct service to patients selecting service types compatible with assessment and identified adherence goals
• Identifies, selects, accesses, and makes referrals to appropriate community resources
• Participates in case conferences, staff meetings
• Maintains appropriate documentation as defined by hospital policies and procedures and program requirements
• Sets appropriate priorities and responds to follow-up situations in acceptable time period
• Demonstrates ability to make decisions, solve clinical problems and manage resources
• Demonstrates sensitivity to patient's cultural background
• Understands the pertinent principles, concepts, values and relationships associated with a given task
• Demonstrates knowledge of HIV treatment. Demonstrates sensitivity to patient's spiritual needs
• Assists in data collection efforts for evaluation and research purposes. Retrieves CLIMACS data on patients as needed.

Skill:

• MS Office, Customer Service, Project coordination experience working in a health-related project preferred.
• Facility with case management.
• Bilingual (Spanish/English) preferred.
• Experience in HIV/AIDS preferred

Education: BA degree preferred or four-five years professional experience
