Job Description

Role: Community Health Advocate
Location: New York, NY
Duration: 13+ Weeks
Guaranteed Hours: 37.5 Hours
Hours per day: 7.5
Shift: 8:30am-5:00pm
Pay Rate: Local - $18.00-$21.00/Hour

Roles & Responsibilities:

Provides direct service to patients selecting service types compatible with assessment and identified adherence goals
Identifies, selects, accesses, and makes referrals to appropriate community resources
Participates in case conferences, staff meetings
Maintains appropriate documentation as defined by hospital policies and procedures and program requirements
Sets appropriate priorities and responds to follow-up situations in acceptable time period
Demonstrates ability to make decisions, solve clinical problems and manage resources
Demonstrates sensitivity to patient's cultural background
Understands the pertinent principles, concepts, values and relationships associated with a given task
Demonstrates knowledge of HIV treatment. Demonstrates sensitivity to patient's spiritual needs
Assists in data collection efforts for evaluation and research purposes. Retrieves CLIMACS data on patients as needed.

Skill:

MS Office, Customer Service, Project coordination experience working in a health-related project preferred.
Facility with case management.
Bilingual (Spanish/English) preferred.
Experience in HIV/AIDS preferred

Education: BA degree preferred or four-five years professional experience
