Job Title: Certified Nursing Assistant (CNA) - Hospice Home Care

Job Description:

As a Certified Nursing Assistant (CNA) with Superior Hospice, you will play a vital role in delivering personalized and compassionate care to patients in their homes. You will work closely with our interdisciplinary team to support patients and their families during a sensitive and challenging time.

Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
  • Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
  • Administer medications and treatments as directed by the RN or LPN.
  • Monitor and record patient vital signs and report any significant changes to the nursing team.
  • Assist in maintaining a clean and safe environment for patients.
  • Provide emotional support and companionship to patients and their families.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Participate in team meetings, case conferences, and in-service training sessions as required.
  • Adhere to all company policies, procedures, and regulatory requirements.

Qualifications:

  • Valid Certified Nursing Assistant (CNA) certification in the state of Minnesota.
  • Previous experience in hospice, home care, or a related healthcare setting preferred.
  • Compassionate and empathetic demeanor with excellent communication skills.
  • Ability to work independently and as part of a collaborative team.
  • Strong organizational skills and attention to detail.
  • Reliable transportation and a valid driver's license.
  • Ability to pass a background check and drug screening.
