Palliative Care Social Worker - PRN

Qualification and Education Requirements

Bachelors or Master’s Degree in Social Work from a State accredited program
License in Social Work in the state you will be working
Valid driver’s license and car insurance
One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

Essential Duties

Assess patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals
Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities related to legal and fiscal issues
Facilitate community resources based on patient/family needs
Participate in all mandatory staff in-services and meetings
Maintain compliance with internal annual education requirements
Timely submission of all documentation
Conduct all business activities in a professional and ethical manner
Performance of other duties as required
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
