Job Description



Full time, day shift (8:00a-4:30p)

General Summary

Under general supervision, provides and coordinates the care provided to transplant patients in the post transplant phase of care. May provide care to other patients as necessary. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness. Is accountable to patients and families for the processes and outcomes of care during the post-transplant phase of care. Is accountable to peers for the advancement of group practice within the Department of Transplantation.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

1. Provides comprehensive patient care to and coordinates multidisciplinary care for solid organ post transplant patients.

A. Provides patient education and pertinent transplant information to post transplant recipients.

B. Coordinates meetings with the nurses and patients to review the treatment plan (inpatient discharge rounds).

C. Monitors patient progress and answers questions related to their post-operative care.

D. Participates in discharge planning with the multidisciplinary team.

E. Orders medication bags from pharmacy.

F. Notifies transplant team members of post-transplant admissions, including admitting personnel, transplant resident, referring physician, and patient placement center.

G. Coordinates scheduling of post hospitalization medical procedures, diagnostic testing, as well as clinic visits.

H. Coordinates data collection post for post-transplant recipients. Completes United Network of Organ Sharing (UNOS) forms required for post-transplant.

I. Coordinates completion of FMLA paperwork for patient or patient’s family.

J. Calls patient 24 hours post discharge to assess any complications.

K. Coordinates stent removal with urology.

Qualifications



Education and Experience

1. Licensure as a Registered Nurse in the State of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required. BSN preferred.

2. Dialysis, Critical Care or Transplant experience is preferred.

3. Must obtain NATCO CCTC certification within 2 years of hire, or within a time frame determined by management. Must maintain NATCO certification once received.

Knowledge, Skills and Abilities

2. Demonstrates ability to plan, organize and follow through on assignments and/or projects. Ability to analyze and conceptually develop ideas and proposals.

3. Ability to learn and use computer systems in order to manage patient information.

4. Demonstrates highly effective interpersonal, verbal and written communication skills.

5. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient’s requirements relative to his or her population-specific and age specific needs.

Patient Safety

Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

1. Takes action to correct observed risks to patient safety.

2. Reports adverse events and near misses to appropriate management authority.

3. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
